If you have experienced or suspect you may have experienced a side effect related to a Kotra Pharma medicine or health supplement, please notify us immediately. To help us review, investigate and respond promptly, please provide the following information using the form below:
1. Affected Individual's Details – initials or identifiers, age, gender and ethnicity.
2. Product information – name of the product involved.
3. Side effect details – a description of the signs and symptoms experienced, the date they began and the current status of the side effect (fully recovered, recovering, not yet recovered or fatal).
4. Reporter's contact information – so we can follow up if needed.
